Aerial & Scissor Lift Evaluation Form

OSHA requires that “site specific” and “equipment specific” safety considerations are always addressed. That’s why no outside classroom training or consultant can provide you the necessary training for your operators at your site and on your equipment.   Add additional questions based on site/equipment specific issues.  Hands-on evaluation portions of the training can also be conducted by an employee in the department/work unit who is experienced and competent with the aerial/scissor lift.
